The official ID shall bear the name, signature and employee number of crew, signature of authorized company representative at the cost of the allottee.

• Allottee will fill-up CFR Service Request Form.
• Submit the CFR Service Request Form together with his / her ID Issuance Authorization for Allottee Form to CFR Staff.
• Upon check up and signature by the CFR staff, allottee will proceed to Magsaysay Coop Office to process his /her ID.

Note:
His / her ID is valid for 2 years provided he / she remains the allottee during the period and the crew’s status is not changed to “Not Reporting” or “Rejected” before the expiration date.
